membership termination / pause

Please use this form to apply for membership termination or pause. Membership pause refers to memberships which are terminated temporarily. Do note that any fees already paid are non-refundable.

Reinstatement Fee

A Reinstatement Fee of $100 is applicable, if a membership is terminated temporarily, and resumes at a later date. This fee is waived if the membership pause is for medical reasons.
